Information Request

Procedures for Requesting Information, Submitting Objections, and Submitting Dispute Resolution Requests to The Information Commission

Every Person has the right to obtain Public Information in accordance with the provisions of the Law.

Every Person has the right to:

a. see and know Public Information;

b. attend public meetings that are open to the public to obtain Public Information;

c. obtain a copy of Public Information through a request in accordance with this Law; and/or

d. disseminate Public Information in accordance with laws and regulations.

 

PROCEDURES FOR REQUESTING INFORMATION

Every Public Information Applicant has the right to submit a request for Public Information along with the reasons for the request. The following are the requirements for a request for public information at Marzoeki Mahdi Hospital.

  1. Requests for Public Information can be made in writing or not in writing.
  2. Applicants are required to include valid Applicant Identification, namely:
    1. Photocopy of Identity Card, Passport or other valid identification that can prove that the Applicant is an Indonesian Citizen; or
    2. Photocopy of the first and last pages of the Articles of Association which have been approved by the Minister of Law and Human Rights and have been recorded in the State Gazette of the Republic of Indonesia (in the case where the Applicant is a Legal Entity); or
    3. Power of attorney and photocopy of the Identity Card of the person granting the power of attorney if the Applicant represents a group of people; or
    4. If the Applicant is accompanied or represented by a power of attorney, the Application must be accompanied by a power of attorney.
  3. If the application is submitted in writing, the applicant fills out an application form;
  4. The PPID Secretariat Team is required to ensure that the application form that has been given a registration number as proof of the Public Information request is submitted to the Public Information Applicant.
  5. Information requests will be followed up after all applicant requirements are received.

PROCEDURES FOR SUBMITTING OBJECTIONS

  1. The applicant fills out an objection form regarding a request for public information.
  2. PPID officers at Marzoeki Mahdi Hospital verify the completeness of the objection submission files for requests for public information.
  3. PPID officers register objections to requests for public information by providing proof of objection.
  4. The objection to the request for public information was addressed to the PPID superior of Marzoeki Mahdi Hospital.
  5. The PPID Head of Marzoeki Mahdi Hospital will provide a response to the applicant no later than 30 working days from the time the objection to the request for public information is registered.
  6. If the applicant is satisfied with the response to the objection, then the public information service is complete.
  7. If the applicant is not satisfied with the response to the objection, then within 14 working days after the response , he can submit a public information dispute application to the Information Commission.

PROCEDURES FOR SUBMITTING A DISPUTE RESOLUTION APPLICATION TO THE INFORMATION COMMISSION

Submission of a public information dispute to the Information Commission must be submitted by the Public Information Applicant no later than 14 WORKING DAYS from the receipt of a written response to the letter of objection from the public information applicant to the superior of the PPID of the public body or the end of the 30 WORKING DAY period for the superior of the PPID of the public body to provide a written response to the letter of objection from the applicant for public information;

  1. Submission of public information disputes by individuals, legal entities or groups of people can be submitted by visiting the information commission office directly and meeting the information dispute resolution and administration officer or submitting a public information dispute application online, such as by email;
  2. Applicants for public information disputes are required to complete the application files for submitting public information disputes before obtaining a registration number/public information dispute registration certificate from the information commission registrar's office officer;
    1. The applicant fills out the Information Dispute Resolution Application Form provided by the officer;
    2. Bring proof of the letter of request for information to the Public Agency and the receipt;
    3. Bring proof of response to information request from Public Agency along with receipt (if any);
    4. Bring proof of submission of objection to the Public Agency and receipt;
    5. Bring proof of the objection response from the Public Agency along with the receipt (if any);
    6. Bring proof of identity (valid identity, namely a photocopy of the Population Identification Card, Passport or other valid identity that can prove that the Applicant is an Indonesian citizen; or 2. articles of association that have been approved by the Minister of Law and Human Rights and have been recorded in the State Gazette of the Republic of Indonesia in the case of the Applicant being a Legal Entity. 3. Power of attorney and photocopy of the Population Identification Card of the person granting the power of attorney in the case of the Applicant representing a group of people).
  3. After the public information dispute application has received a registration number or registration certificate, 14 working days later the information commission will begin the process of resolving the public information dispute by starting by properly summoning the applicant and respondent to attend the non-litigation adjudication hearing at the initial examination stage.
